Chevron refinery explosion |

Ivan Vince has been interviewed on BBC Wales Radio and TV following the explosion on 2 June at the Chevron refinery in Pembrokeshire.
The explosion claimed the lives of four contractors who were apparently carrying out routine maintenance on an empty storage tank. As the HSE and Chevron investigations get under way, a fifth person remains critically ill in hospital.
Asked about the likely course the investigations would take, Ivan explained that the search for immediate causes - e.g. failure of safety devices and other equipment, operator error, presence of ignition sources - would be followed by a broader and deeper analysis of root causes, looking at the adequacy of the safety management system as a whole and the safety culture prevailing at the installation. He was very confident that useful lessons would be learned; but less so that the same lessons would not need to be re-learned in future, as often happens, unfortunately.
